    $46 per patient per month is the avg reimbursement for CPT 99490.  

    As of January 2015, Medicare began reimbursing CPT code 99490 (2017 average of $46.00 ppm) for providing Chronic Care Management services. In 2017 Medicare expanded the program to include a host of other reimbursable codes designed to promote improved patient outreach and engagement. The aim of the program is to reduce ER visits, hospitalizations, and readmissions.
