AWV FAQ's

 

How are patients selected for home visits?

Criteria are based on your guidelines, such as:

- No office visit in the past 12 months

- Frequent use of emergency room visits

- Other standards to address specific gaps in care

This home visit program is available to Medicare and Medicare Advantage patients.

 

Can patients opt out?

Yes. This is a completely optional program

 

To participate, do patients need to sign any type of consent?

Yes. A consent form can be signed prior to the visit, or at the home before the assessment begins. You may use your own consent form or one of ours.

 

Does a physician conduct the Annual Wellness Visits (AWV) / Health Risk Assessment?

Medicare Annual Wellness Visits (AWV) are performed by Doctors, Physician Assistants, Nurse Practitioners, CRNA’s and MA’s.

 

What type of outreach will be used to contact these patients?

A coordinated effort that includes customized direct mail, phone interviews and scheduling — all provided by HRA IQ™.

 

If new or acute issues are discovered during the home visit, how are patients directed back to the Healthcare System?

HRA IQ™ clinicians or providers will not script or order tests; HRA IQ™ represents itself and acts as an extension of your practice. All findings are directed back to you, the provider, for follow-up and continued patient retention.

 

What about ordering labs and imaging? Who receives and signs off on the results?

HRA IQ™ clinicians or providers will not order labs or imaging. We may provide recommendations for these tests as part of the follow-up report or 5-year plan provided after each patient visit/assessment.

 

How are Annual Wellness Visit findings communicated back to the system?

The 5-year plan, all questionnaires/answers and a summary of procedures (with associated billing codes) will be communicated back electronically.

 

Will we be able to use these visits to update our EHR?

Yes. We will work with you to determine your platform for electronic transfer of all captured data.
